Patient Safety Care: How do we compare?
At Community Hospital, we take pride in the care we provide. To monitor the quality of that care, we track specific quality measures and compare them to benchmark measures. We publish this information so that you can see how effective CHA is with patient safety.

The Institute of Medicine published a report in 2000 that highlighted the stunning effects of medication errors. The report sets forth a national agenda for reducing medical errors and improving patient safety by designing a safer health system. Although CHA's medication error rate is better than the national benchmark, we make continuous efforts to improve the process. CHA should be at or below the national benchmark. |

Protecting patients from hospital-acquired infections is a primary patient safety goal. CHA has an ongoing program to prevent and treat infections in patients. Per the Centers for Disease Control recommendations, CHA tracks high-risk patients. This includes patients with increased exposure to infection due to the presence of ventilator, catheter, central venous line or other invasive device. The infection rates below are for Intensive Care Unit patients. |

Last May study published in the New England Journal of Medicine reported that patients are safer and less likely to experience serious complications when they are treated in hospitals with more registered nurses on staff.(3) CHA recruits clinical graduates from area nursing schools for open positions at the hospital. CHA should be at or below the state benchmark. |

Patient Safety is a high priority goal at CHA. We want our patients to feel safe when they are in the care of our hospital. CHA strives to make the environment as safe as possible for our patients. CHA should be above the goal. |

There are many reasons for unscheduled returns to a hospital such as inadequate discharge planning, patients not following the discharge instruction or patients not provided with needed education. Some returns are unavoidable, CHA monitors the frequency of these returns. CHA should be at or below the goal. |

CHA completes a comprehensive evaluation of patients and their risk of falling while in the hospital. Bed alarms, personal alarms and the use of sitters with the patient help in decreasing falls within the hospital. CHA should be at or below the goal. |
